§ 42-251. Permit application for abattoir.

(a)

Any person desiring to operate an abattoir shall make written application for a permit on forms provided by the health authority. Such application shall include the following:

(1)

The applicant's full name, post office address, street address;

(2)

Whether such applicant is an individual, firm, corporation and if a partnership the name of each partner and his address;

(3)

The location and type of the proposed abattoir; and

(4)

The signature of the applicant.

(b)

If the application is for a temporary abattoir, it shall also include the inclusive dates of the proposed operation.
